The Power of Chronic Disease Self-Management Programs

Managing a chronic disease can feel overwhelming, but it doesn’t have to define your life. Chronic Disease Self-Management Programs (CDSMPs) empower individuals to take control of their health, offering a structured yet flexible approach to improving quality of life.

What Are Chronic Disease Self-Management Programs?
