The Mid-MO SMBP implements the Healthy Heart Ambassador Blood Pressure Self-Monitoring Program (HHA-BPSM) across Boone and Cole Counties

What is the Mid-MO Self Management of Blood Pressure Program?

The Mid-MO SMBP is facilitating the Healthy Heart Ambassador Blood Pressure Self-Monitoring Program (HHA-BPSM) in Boone and Cole counties.

This program:

  • Is evidence-based, lasting four months for each enrolled participant
  • Includes 10-minute consultations with a program facilitator during drop-in office hours
  • Provides weekly check-ins from the program facilitator by phone, email, or text, and monthly nutrition education seminars.
  • Invites participants to attend two personalized consultations per month (office hours) as well as monthly nutrition education seminars.

HHA-BPSM-trained program facilitators work with participants under the supervision of an HHA-BPSM trained manager and overall program coordinator.

Nutrition Education Seminars included within this program are:

  1. Dietary Approaches to Manage Hypertension
  2. Reducing Sodium Intake
  3. Shopping, Preparing and Cooking Food for Better Blood Pressure Management
  4. Heart Healthy Eating for Life
